Case Background

Investigation Situation

Our company had boarded the ship for investigation on September 9th, 2021, and had found that:
1. Wires of crane had broken, and obvious extrusion and friction marks can be seen on the unbroken wires;
2. The crane pully had damaged because of the impact, and visible bending could be seen on its side plate with the bending part having friction with the pully;
3. Obvious bending could be seen on the feet of the cargo hatch girth and the deck;
4. The wallboard of the cargo hatch had bent and deformed due to the impact;
5. Obvious friction scratches had been found inside the hatch. After verifying the lifting load, we had found that the total lifting weight exceeded the total load of the crane (overload lifting), and the accident was caused by the improper operation of the loaders.

Investigation Situation

Solution
